Position Overview:
The Parent Child Assistance Program Mentor works within a multi-disciplinary team to provide targeted prevention and outreach to families most at risk of having children affected by FASD. The PCAP Mentor will work cooperatively with mothers who are at risk of drinking while pregnant to develop individualized action plans with clearly defined goals and measures of success to maintain healthy lifestyles and prevent FASD affected births. The PCAP Mentor will follow the program guidelines of the Alberta PCAP council. The PCAP Mentor is required to facilitate community engagement, collect and manage privileged information, attend to administrative responsibilities and provide general support to the Tawow Community Centre.
Duties:
- Provide services to targeted PCAP clients using the PCAP Model for service delivery.
- Establish and maintain relationships with government departments and agencies providing services and programs to PCAP clients.
- Ensure home visitation activities address service needs across a lifespan and are respectful of a wide range of perspectives.
- Collaborate with community agencies and provide supported referrals, to address problems such as homelessness and addictions faced by individuals in the program.
- Develop and Implement Preventative Social programs and activities for PCAP clients, new and expecting mom’s and women of childbearing age.
- Implement best practices into the supports and services offered.
- Meet monthly with key community stakeholders such as interagency and AWN committees.
- Maintain an office at the AWN-Tawow Community Centre, including but not limited to purchasing supplies, scheduling, correspondence, filing, fielding telephone calls.
- Collect program statistics and enter data for Quarterly, Bi-annual and Annual Reporting.
- Write a monthly report to the Board of Directors from the PCAP program.
- Attend staff meetings, staff training and complex care team meetings.
- Attend Tawow Community Centre’s annual strategic planning meeting, identify priorities and contribute to finding solutions.
- Complete Tawow Client Program Admission and assessments with each new client.
- Document all client interactions in a timely manner.
- Evaluate the effectiveness of Tawow Community Centre PCAP programs and assess referral programs’ suitability for clients.
- Arrange transportation for clients to and from medical and social support appointments.
- Travel for client services, meetings, conferences, and workshops as necessary.
- Ensure practice supports the confidentiality and privacy of clients, AWN, staff, volunteers, and stakeholders and follow FOIP regulations.
- Perform assignments/other duties as required by the Preventative Social Programs Manager.
Qualifications:
- Diploma or Degree in Human Services, Social Work or related field preferred. An equivalent combination of education and experience may be considered.
- One – two years’ experience working with individuals with individuals in Human Services and/or home visitation.
- Valid Class 5 Alberta Driver’s Licence and clean driver’s abstract.
- Advance level PC skills as well as general computer email, internet knowledge.
- Proficient in the use of Microsoft Office Suite – Word, Excel, Power Point.
- Clean Vulnerable Sector Criminal Record Check.
- First Aid & CPR Certification.
- Mandatory Drug & Alcohol testing.
Work Demands:
The position is primarily sedentary with occasional lifting up to ten pounds.
Environmental Conditions:
The incumbent is located in a busy office environment, will attend meetings in the community and travel outside of the community as required.
